Must be a Dekalb County Resident.

HOURS
Tuesday 10am-4:00pm
Thursday 10am-4:00pm
Monday,  Wednesday, & Friday – by appointment.

We assist individuals with past due utility bills, rent, transportation to medical appointments, and non-narcotic prescription medications.
PAPERWORK NEEDED

    Client must bring a valid Photo ID
    Social security numbers for all living in the home
    Most recent bill
    Filled out application and questionnaire
    Filled out affidavit (Rent and mortgage assistance only)
    Paperwork from Township Trustee

*Special Note*

Due to funding, we require bills be lower than $150.00 before we will consider taking a closer look at your application. This does not mean we can assist you with the entire $150.00.

 

What you need to do/bring:
Fill out questionnaire
Fill out application (front and back)
    *Please note that filling out the application does NOT guarantee assistance*
Bring a picture ID
Bring Proof of Address
Bring verification that you had an appointment with the Township Trustee
Bring SSN’s of all individuals living in the household
Bring a filled out Affidavit
    *For rent assistance only*

Bring proof your bill is under $150.00
    *Please note that this does NOT mean we will be able to pay the entire $150.00*

Tips For Prompt And Professional Assistance
1. Make an appointment - Always attempt to make an appointment. Oftentimes this will save you hours waiting in lines. When on the phone make sure you ask for the documentation needed in order to receive assistance.

2. Do not be late for your appointment - This tip should speak for itself.

3. Dress appropriately - If you want to be treated with respect, dress as appropriately as possible. We are not saying wear a suit. Just make sure you and your clothes are clean and non offensive. We have found that clients who dress in this manner have a better chance of receiving aid.

4. Bring all pre determined documentation - Bring all of the documents you were advised to bring from your appointment phone call. Without them, you will not receive aid.
